In this video, spinal stenosis procedure is shown. Spinal stenosis is a medical condition in which the spinal canal narrows and compresses the spinal cord and nerves. This is usually due to the common occurrence of spinal degeneration that occurs with aging. It can also sometimes be caused by spinal disc herniation, osteoporosis or a tumor. In the cervical (neck) and lumbar (low back) region it can be a congenital condition to varying degrees. Spinal stenosis may affect the cervical, thoracic or lumbar spine. In some cases, it may be present in all three places in the same patient.
Symptoms of spinal stenosis
Numbness
Cramping
Pain in the back, buttocks, thighs or calves
Weakness of a portion of a leg or arm
Causes of spinal stenosis
Arthritis involving the spine, usually in middle-aged or elderly people
Herniated or slipped disk
Injury that causes pressure on the nerve roots or the spinal cord itself
Defect in the spine that was present from birth (congenital defect)
Tumors in the spine
Various types of cervical fusion procedures are-
Anterior Cervical Fusion – It is a neurosurgical procedure that is performed to relieve the pressure on one or more nerve roots, or on the spinal cord. It involves the stabilization of two or more vertebrae by locking them together. The fusion stops the vertebral motion and as a result, the pain is also stopped. Anterior refers to the front meaning that the surgeon reaches the cervical spine through a small incision in the front of the neck, usually within a skin fold line under the chin. The nucleus pulposus of the disc bulges out through the annulus and presses on the nerve root next to it. This nerve root becomes inflamed and causes serious pain. The disc consists of about 80 % water. When one grows older, the disc starts to dry out and shrink, causing small tears in the annulus and inflammation of the nerve root. After retracting the neck muscles, the neurosurgeon uses an operating microscope and removes the affected intervertebral disk, which takes the pressure off the nerves or spinal cord. This is called decompression. He then replaces it with a bone graft that will fuse the vertebrae together over time. A bone graft is used for the fusion between the two bones. It is obtained from a bone bank, which collects, treats, and stores bone. The bone comes from human donors who are recently deceased. This type of graft is referred to as allograft bone and ids without blood cells. The preformed bone plug is safe and will not be rejected by the patient’s body. In some cases, the neurosurgeon may use metal plates or pins that provide extra pressure and support to the neck and to make sure that the bones fuse adequately. The length of surgery depends upon the number of vertebrae involved.
Anterior Interbody Fusion - This type of fusion is much more common in the neck. This type of fusion is the same as spinal stenosissurgery. In the interbody fusion, a bone graft is placed between two vertebrae that replaces the removed disc. During the healing process, the vertebrae grow together, creating a solid piece of bone out of the two vertebrae.
Posterior Fusion - In the posterior fusion, the bone graft is placed on the back side of the vertebrae. During the healing process, the vertebrae grow together, creating a solid piece of bone out of the two vertebrae. This type of fusion is only rarely used in the spinal stenosissurgery, generally only for fractures of the spine. The anterior interbody cervical fusion is used to treat most problems in the neck caused by degenerative disc disease such as unrelieved neck pain and pressure on the nerve roots caused by bone spurs or a herniated disc. Posterior fusion is generally performed to stop the motion caused by segmental instability reducing the mechanical neck pain caused from excess motion in the spinal segment.
Instrumented Cervical Fusion – In this type of cervical fusion, the bone graft is simply wedged in between the vertebra. It is held there simply because it is wedged in tight with the help of metal plates, screws, and rods to try to increase the success of helping the spine to fuse. In this type of fusion the vertebra is held in such a way that it allows no motion in between them due to which the bones heal faster. In this type of spinal stenosissurgery, the most common form of internal fixation is using a metal plate and screws. The plate sits on the front of the vertebrae and the screws go backwards into the vertebral body to help hold the plate in place and to help keep the bone graft from slipping out of place. By using metal plates and screws, the vertebra can be held rigidly in place while the fusion heals. Braces and casts are not needed.
